Care Manager Visit
Your live-in care manager will visit you or your loved one at home or in hospital
Establish Your Care Needs
We will establish your care needs, meet your family and your pets, and develop a full understanding of what is required
Funding Options
Together, we can discuss funding options. We will introduce your Vitality Home Health financial practitioner, who can give you information on claims and assistance with fees
Your Care Plan
We will write up a care plan that draws all of this information together Based on your needs, we will suggest and discuss staff members who are available to provide your care, introducing them or showing you their profiles
